CA Identification Number - Application - Help

Important:

Failure to complete the application accurately may delay processing of the application and the assignment of your CA Identification Number.

Registration Fee:

• A one-time payment for the registration fee of one hundred dollars ($100.00) is payable upon application by MasterCard, Visa or Amercian Express.
• Instructions on how to pay using your credit card are provided once you select the "Submit and pay" button in the application form.

Dealer Name:

• Only one name under which the dealer normally carries on business should appear on the application.
• Where the company does not have a legal business name, the applicant's name or any other legal identity (e.g. 1234 Canada Ltd.) may be used until such time that a legal business name has been obtained.
• Notification of any change of name must be made to the Information Centre of the Competition Bureau.

Principal Place of Business:

• Must be a physical location of business or corporate office in Canada (i.e. street address, city (do not abbreviate), province, and postal code). A box office is only acceptable as a mailing address, in addition to your physical address.
• Must be written in accordance with the Canada Post Guidelines.

Note: Where the principal place of business and mailing address differ, both addresses must appear.

Canadian Mailing Address:

• Must be written in accordance with the Canada Post Guidelines.
• If applicable, include postal box number and postal station, general delivery information, rural route number, etc.

Authorized Official in Canada:

Identity of an authorized contact person that is responsible for:

• the CA Identification Number application and modifications;
• the products to which the CA Identification Number will be applied;
• the consumer inquiries and complaints.

Please do not call regarding the status of your application. We will mail you a letter, advising you of the CA Identification Number that has been issued to you, as soon as the application and payment have been successfully processed.
